2. for medical practice supplies, code 9
3. Description of medical practice supplies and quarter
4. Health insurance fund IK number
5. 9-digit practice location number (BSNR)
6. 9-digit contract physician number (LANR)
7. Date of issue
8. Enter the name of the medical practice supply item with its PZN (pharmaceutical central number; see online catalog), exact size and quantity details, max. 3 items, additional prescriptions if applicable
9. Contract physician's stamp and signature (if there are several physicians, the signature must match the LANR!)
